Muscle strains: the timeline and how not to rush it
A pulled hamstring or a tweaked pec follows a fairly predictable course. Most of the bad outcomes come from returning to full load before the tissue is ready.

A muscle strain is a tear of muscle fibres, ranging from a small number of fibres to a complete rupture. In lifters they occur most often in the hamstrings, the pectorals, the adductors and the calves, usually during a rapid or heavily loaded eccentric.
General information. Anything with a loud pop, immediate severe weakness, a visible deformity, or extensive bruising should be assessed promptly, since complete tears sometimes require surgical repair and the window for that can be limited.
The grading, roughly
Grade one: a small number of fibres, mild pain, minimal strength loss, full range preserved. Usually settles over a week or two.
Grade two: a partial tear, noticeable pain and weakness, some loss of range, often bruising. Weeks to a couple of months.
Grade three: complete rupture, severe pain at the time, marked weakness, often a palpable gap. Requires assessment; some are managed surgically.
These categories are broad and the timelines vary substantially with the muscle involved and the individual.
The early phase
For the first few days: protect the area from further damage, keep moving within a comfortable range, and manage pain enough to allow gentle activity.
The old advice of complete rest and prolonged icing has been substantially revised. Extended immobilisation produces more scar tissue and a weaker repair. Gentle early movement, within pain limits, appears to produce better outcomes.
Ice may reduce pain and there is limited evidence that it improves healing. Compression and elevation for swelling are reasonable.
There is ongoing debate about anti-inflammatory medication in the very early phase, with some evidence that it may impair the initial healing response. Occasional use for pain is fine; routine high-dose use throughout recovery is questionable.
The loading phase, which is where recovery happens
Once the acute phase settles — usually within days for a mild strain — progressive loading is the intervention that matters.
The general progression: isometric holds in a comfortable position, then movement through range without load, then light loading through a partial range, then progressive load through the full range, then eccentric-focused work, then speed and power if relevant.
Each stage progresses when the previous one is tolerated without a return of symptoms. Pain up to a mild level during rehabilitation exercise is generally acceptable; sharp pain or next-day worsening means the dose was too high.
Eccentric loading specifically has the best evidence for hamstring injuries in particular, both for rehabilitation and for prevention. Exercises that load the muscle in a lengthened position appear to be protective, and the Nordic hamstring exercise has a substantial body of evidence behind it for reducing hamstring injury rates.
Why people reinjure
Reinjury rates after muscle strains are high, and the reasons are consistent.
Returning too early. Pain resolves before capacity returns. Feeling fine is not evidence that the tissue tolerates full load.
Insufficient strength at end range. A muscle that is strong in the middle of its range and weak in the lengthened position is vulnerable, and the lengthened position is where strains happen.
Not addressing what caused it. A sudden increase in load or sprinting volume, insufficient warm-up, accumulated fatigue, or an existing imbalance.
Skipping the final stages. Rehabilitation that stops when normal activity is comfortable, without ever returning to high-speed or heavy loading, leaves a gap between what was rehabilitated and what the sport demands.
Return criteria worth using
Rather than a fixed number of weeks:
Full pain-free range of motion compared with the other side. Strength within roughly ten percent of the uninjured side, tested through the range including the lengthened position. The ability to perform the movements the activity requires at speed without apprehension. And no pain the following day after a full training session.
Apprehension is worth listening to. People who report that a muscle does not feel trustworthy frequently turn out to be right.
Prevention
Progressive load management — most strains follow a spike in training load. Adequate eccentric strength through full range. Proper warm-up before high-intensity or high-speed work. Adequate sleep, since fatigue is an established risk factor.
And a previous strain is the single strongest predictor of a future one, which is the best argument for completing rehabilitation properly rather than stopping when the pain does.
